Job Summary: The Healthcare Provider will be responsible for conducting physical and mental/behavioral health assessments to military service members in Alaska. Acting as a subject matter expert regarding physical and behavioral health requirements and regulations, the Health Care Provider will perform Periodic Health Assessments (PHA), Mental Health Assessments (MHA), Pre and Post Deployment Health Assessments and Post Deployment Health Re-Assessments (PDHRA).
Essential duties and responsibilities:
- Conduct Physical and Mental Health Assessments (PHA + MHA) and Post Deployment Health Re-Assessments (PDHRA) in all 50 states through telehealth portal in a remote call center environment.
- Triage and manage client contact including referrals to other resources if necessary.
- Complete required documentation associated with all PHA, MHA, and PDHRA assessments.
Education and Qualifications:
- Must be a licensed Health Care Provider (HCP) who is a Nurse Practitioner (NP) or Physician Assistant (PA).
- Specialized Master’s degree or higher from accredited institution in specialty area
- Active Accreditation and/or licensure for specialty area
- Minimum 3 years’ experience providing mental and behavioral health services/assessments.
- Ability to work effectively in a remote setting with little supervision, while achieving program goals.
- Experience working with electronic health records and databases, as well as Microsoft Office Suite.
- Excellent organizational and multi-tasking abilities.
- Must have the ability to obtain SF86 Position of Trust.

Hours and Shift:
- PT Available
- 1099 Contract
- Monday-Friday 6:00am-9:00 pm
Travel:
- Less than 10%

Physical Demands: Works in remote office environment, sits, stands, bends, lifts and moves intermittently during working hours. Uses standard computer equipment. The employee must occasionally lift and/or move up to 10 pounds.
