Job Summary:
Aptive Resources is seeking a highly skilled and compassionate Advanced Practice Professional (Nurse Practitioner) to support the National Teleoncology Division for the Veterans Health Services. The ideal candidate will have a strong clinical background in oncology, exceptional patient care skills, and the ability to work effectively in a collaborative healthcare environment. This position requires a current, unrestricted nursing license, graduation from an accredited nursing program, and relevant certifications
Primary Responsibilities:
- Provide care via telehealth modalities: The TeleOncology Nurse Practitioner (TONP) delivers health care across the continuum of care within the service, is responsible for care coordination and quality of care, and has a specific focus on the population for “time to treatment” related to Cancer Screening, diagnosis, treatment, and follow-up surveillance. This includes relatively independent functioning and ongoing collaboration with attending physicians, peers, and other healthcare team members. The Contractor’s staff shall see approximately six (6) patients per day via phone, Clinical Video Telehealth (CVT), or VA Video Connect (VVC).
- Oncology Expertise: The TONP’s role as an advanced practitioner is characterized by the autonomy of hematology and oncology practice, in-depth nursing knowledge, and evidence-based expertise. Core clinical functions include performing history and physical examinations, diagnosing, treating, and managing acute and chronic health care problems, ordering appropriate testing and consults, performing adult care check-ups, prescribing medication, and treatments, and teaching patients’ health promotion and disease prevention. The Contractor’s staff shall demonstrate this in the clinical notes and peer observation.
- Process incoming consults by assigned team and subspecialty diagnoses: The Contractor’s staff shall follow the Standard Operating Procedure (SOP) titled Team Consult Care Coordinator Registered Nurse (CCRN) Procedures, CLO 201, dated 5/22/2024, to handle all incoming consults to the assigned team. More than three (3) deviations from SOP guidelines in a quarter are considered unacceptable.
- Process individual view alerts in CPRS: Contractor’s staff shall process view alerts no later than seven (7) days from the date they were created.
- Complete clinical progress notes in CPRS: The Contractor’s staff shall complete all clinical notes within 48 hours of the patient’s visit. The Contractor’s staff shall have no more than two (2) unsigned progress notes greater than 48 hours per week.
- Customer Service: The Contractor’s staff shall consistently demonstrate Integrity, Commitment, Advocacy, Respect, and Excellence (ICARE) values. The Contractor’s staff shall have no more than two (2) written, valid, substantiated complaints from internal or external customers per quarter.
- Follow all established medication policies and procedures. No sample medications shall be provided to patients.
- Provide discharge education and follow up instructions that are coordinated with the next care setting for all Hematology/Oncology clinical patients.
- Participate in continuous quality improvement activities and meetings with committee participation as required by the facility Chief of Service, Chief of Staff, or designee.
- Attend staff meetings as required by the facility Chief of Service, Chief of Staff, or designee
- Utilize broad-based clinical knowledge to assess, implement health promotion and prevention interventions, manage acute and chronic oncology illness states, and assist patients/residents in attaining optimal levels of functioning and rehabilitation.
- Evaluate patient/resident responses to actual or potential health problems and provide supportive measures for patients/residents undergoing oncology treatments and their significant others.
- Maintain patient/resident confidentiality and a safe care environment in accordance with The Joint Commission and other VA regulatory bodies.
- Provide patient/resident-centered care by assessing healthcare needs, developing plans of care, and performing/assigning delegated tasks specific to oncology care.
- Collect and document pertinent data into the electronic medical record, complete nursing assessments within required time frames, and act on significant changes in patient/resident health conditions.
- Update patient/resident care plans, notify physicians of changes in condition, and report incidents of abuse immediately to the Nurse Manager or designated nursing leadership.
Minimum Qualifications:
- Current, unrestricted license from a State, Territory, or Commonwealth of the United States or the District of Columbia.
- Graduate of a professional, accredited nursing school approved by the appropriate State or possession of current, unrestricted license for graduates of foreign nursing schools.
- Basic Life Support (BLS) certification.
- Advanced Cardiovascular Life Support (ACLS) certification is required for work in acute care, med-surg, or critical care areas.
- Must have 3 years of outpatient Hematology/Oncology experience.
- Must have 2 years of Care Coordination/Case Management/Navigation
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment and provide compassionate care to veterans.
Desired Qualifications:
- AONCP (Advanced Oncology Certified Nurse Practitioner) and or ACHPN (Advanced Certified Hospice and Palliative Nurse) would be highly desireable
