Location: 100% Remote

Duration: Contract-to-hire

Training Schedule (6 Weeks): Monday – Friday, 8am – 4:30pm Central Time (CT)

Post-Training/Normal Schedule: Monday – Friday, 9:00 to 5:30pm Pacific Time (PT)

Pay Rate: $20.00 – $24.00/hour, based on experience.

Job Responsibilities:

  • Conduct Chronic Care Management (CCM) Calls: Perform monthly CCM phone calls with patients, practices, pharmacies, etc., spending 7-8 hours daily on the phone.
  • Create and Revise Care Plans: Develop or update personalized care plans for each patient in coordination with supporting practices and providers.
  • Identify Needs and Coordinate Services: Identify social determinants of health, care gaps, and eligibility for assistance or referral services, coordinating with office staff and community resources.
  • Empower Patients: Support patients in managing their wellness and achieving their goals within their care plan.
  • Manage Call Volume: Make approximately 30-50 phone calls daily to achieve 15 patient interactions necessary for completing 20 minutes of CCM service monthly.
  • Ensure Privacy and Compliance: Maintain patient privacy, confidentiality, and safety, adhering to ethical, legal, and regulatory standards.

Job Requirements:

  • Remote Nursing Experience: Minimum one (1) year of experience working in a remote nursing position.
  • Experience with a Geriatric Patient Population: Experience working with an elderly patient population.
  • Licensure: Active LPN/LVN license that is compact/multistate.
  • Bilingual: Fluent in English/Spanish, verbal and written.
  • Communication Skills: Professional phone demeanor, compassionate and engaging tone, and quick thinking skills to respond to patient inquiries.
  • Call Volume: Ability to conduct 50 calls per day to patients.
  • Technical Skills: Ability to navigate and utilize multiple systems and screens simultaneously. Proficiency in technology, including multiple EMR/EHR systems, Microsoft Office, Google Suite, and quick software learning.
