The Role

We are seeking compassionate, detail-oriented Care Managers to support patients enrolled in Chronic Care Management (CCM) and related Medicare programs. In this role, you will serve as a consistent point of contact for patients between visits, helping them stay engaged, addressing care needs early, and escalating issues appropriately to clinical teams.

This is a fully remote role focused on patient communication, care coordination, and high-quality documentation.

Key Responsibilities

• Conduct monthly CCM check-ins with patients via phone, voice, or digital tools
• Support patients with chronic conditions by reviewing symptoms, medications, and care plans
• Identify changes in patient status and escalate clinical concerns per protocol
• Reinforce care plans, medication adherence, and preventive care recommendations
• Coordinate with provider offices for refills, referrals, and non-urgent requests
• Accurately document patient interactions in the platform to support compliant billing
• Track engagement time and activities to meet CMS CCM requirements
• Maintain a professional, empathetic, and patient-centered communication style

• Patients feel supported, heard, and engaged in their care
• High completion rates of monthly CCM touchpoints
• Clear, accurate, and timely documentation
• Efficient triage of issues to the appropriate clinical pathway
• Strong collaboration with internal care teams and partner practices

Qualifications

• Background in healthcare (RN, LPN, MA, care coordinator, or similar experience preferred)
• Experience with chronic disease management or care coordination strongly preferred
• Comfort speaking with patients by phone and documenting interactions in real time
• Strong organizational skills and attention to detail
• Ability to follow protocols and escalate appropriately
• Comfort working with technology and learning new digital tools
• Excellent written and verbal communication skills
• Deep empathy for our patient population

Nice to Have

• Prior experience with Chronic Care Management (CCM), RPM, or care navigation programs
• Familiarity with Medicare populations
• Experience working remotely in a healthcare setting
