Looking for motivated LPN’s ($17/hr to start) or CMA’s ($14/hr to start) with an opportunity for a raise at 90 days in Florida, Georgia, Alabama, Mississippi, Tennessee, Texas, Kentucky, or Oklahoma. Bonus Potential: LPN’s can earn up to $24/hr when reaching full bonus potential and CMA’s can earn up to $17/hr when reaching full bonus potential. Our Chronic Care Management position is a full-time Mon – Fri day-shift work from home, fully remote opportunity. Chronic Care Management (CCM) provides one on one attention via telephone calls to patients with two or more chronic diagnoses. The nurse collects patient health information to identify risks, provide education and encourage compliance of treatment. The Care Manager will be responsible to create a patient specific Care Plan that provides goals and interventions to implement, monitor and support the needs of the patient as defined by the PCP. The CCM program supports the patient to promote the highest level of quality care.

Requirements include:

  • LPN or CMA required
  • Must have high speed internet, not satellite internet or hotspot
  • Must be proficient in Microsoft Outlook and Excel
  • Familiar with documentation in EMR’s
  • Must be self motivated and disciplined

Responsibilities include:

  • Provide initial risk assessment gathering the clinical elements necessary to determine a patient specific Care Plan.
  • Work with patient to plan and monitor their care as determined by the PCP.
  • Promote compliance with PCP/Specialist office visits.
  • Promote compliance with medication.
  • Act as a patient advocate, liaison and information resource.
  • Provide chronic care education for chronic/complex conditions as determined.
  • Provide appropriate health education.

Job Type: Full-time

Pay: $14.00 – $24.00 per hour
