Remote Clinical Case Manager – Fluent in Russian (RN)

***This is a Full-Time + Remote position for applicantsOUTSIDE OF NY ***

We are Looking for Russian Fluent LPNs to operate in a Case management role.

Remote Focus provides Chronic Care Management, Remote Patient Monitoring, and Telehealth services to Medicare members with chronic medical conditions. We are currently seeking a Case Manager to join our team.

The Case Manager plays a vital role in providing case management and remote monitoring for a caseload of patients with chronic health conditions. Essential duties and responsibilities include:

• Follow-up on clinical assessments and care plans to facilitate applicable goals and interventions of the client
• Ensure the patient’s remote monitoring is being measured. Address and escalate any concerning measurements with the medical provider
• Advocate for and support the client while engaging with health care providers on the patient’s behalf with the goal of reducing emergency room visits and/ or hospitalizations
• Provide clinical support by providing consultation, education information around conditions and interventions to maintain focus on outcomes and best practices
• Submit progress notes and documents, records, and other related reports in an organized, timely and accurate manner as per policy and procedure
• Provide outreach and engagement of the patient to ensure all services are provided
• Provide crisis intervention and follow-up
• Perform other related duties as assigned

SKILLS AND/OR EXPERIENCE REQUIRED

• Bachelors OR at least 2+ years experience in related human services providing direct services to patients diagnosed with chronic health conditions or other disabilities OR 2+ years of experience as a medical assistant or related position
• Bilingual language: Russian and English
• Effective interpersonal, communication, and teamwork skills required
• Experience with harm reduction and motivational interviewing techniques strongly preferred

Work Location: Remote for Candidates outside of NY

Schedule: 9 am – 5 pm; M-F

Starting Salary: $50,000.00 +
