Functional Medicine & BHRT – Compact Licensure Nurse Practitioner | Telemedicine

1st Optimal is seeking a highly skilled and forward-thinking Telemedicine Medical Provider to join our elite virtual care team. We specialize in advanced Bio-Identical Hormone Replacement Therapy (BHRT), Longevity Medicine, and evidence-based weight loss protocols, and we are expanding our functional medicine services to meet the needs of high-performing individuals seeking truly personalized care.

This role is ideal for a provider passionate about root-cause medicine, cutting-edge diagnostic testing, and performance optimization. The right candidate will be experienced in concierge-style care and confident managing complex cases related to hormone health, metabolic function, and gastrointestinal balance through comprehensive lab testing and Advanced Diagnostics.

Key Responsibilities:

• Deliver high-quality, patient-centered care through telehealth visits across licensed states.
• Diagnose and manage treatment protocols focused on hormone optimization, metabolic wellness, and performance enhancement, grounded in functional medicine principles.
• Incorporate advanced functional testing into patient evaluations, including:
• GI-MAP and stool testing
• SIBO Testing
• Food sensitivity and allergy panels
• DUTCH hormone metabolite testing
• Full-spectrum thyroid panels and adrenal function assessments
• Interpret labs and craft individualized protocols involving nutraceuticals, peptides, prescription therapies, and lifestyle interventions.
• Educate and empower patients on long-term strategies for vitality, weight management, sexual wellness, and cognitive health.

Qualifications:

• Licensed NP (nurse practitioner) only (MDs and DOs are not eligible for this role).
• Must hold an active DEA license.
• Must have valid compact licensure (e.g., Nurse Licensure Compact – NLC).
• Must have coverage in at least 4 of these states: Connecticut, Idaho, Iowa, Maine, Maryland, Minnesota, Montana, Nebraska, Nevada, New Hampshire, North Dakota, South Dakota, Washington D.C., Wyoming, Utah, Delaware, Kansas, Alabama, Louisiana, Mississippi, West Virginia, Wisconsin
• Minimum of 3 years experience in functional, integrative, or age management medicine preferred. Including but not limited to GI Map, DUTCH lab interpretations and protocols.
• Proficient with EHR systems (Charm EHR experience a plus) and telemedicine platforms.
• Strong communicator with a passion for working with high-performance individuals and optimizing long-term health outcomes.

Experience:

• Previous experience in a telemedicine or virtual care provider role preferred.
• Proficiency in using EHR systems and video conferencing software.
• Experience with hormone replacement therapy, weight loss management, hair loss and sexual wellness.
• Familiarity with concierge medicine practices.
• Strong understanding of performance optimization treatments and strategies.
• Excellent communication and interpersonal skills.

Requirements:

• MUST HAVE valid compact licensure (e.g., Nurse Licensure Compact – NLC)
• Valid Nurse Practitioner (NP) license.
• DEA license for prescribing controlled substances.
• Ability to work independently and as part of a team.
• Strong attention to detail and accuracy in documentation.
