About the Role
CT Health Advocate is seeking an Elder Care Supervising Provider to support older adults through a combination of virtual care, care coordination, and patient advocacy.
This role focuses on working with Medicare and Medicare Advantage patients with complex medical, functional, and social needs. The Supervising Provider plays a key role in bridging gaps between providers, services, and real-life patient challenges that often go unaddressed in traditional care models.
This is a flexible independent contractor role designed for a clinician who values autonomy, critical thinking, and a holistic approach to patient care.
What You’ll Do
- Conduct initiating telehealth visits (E/M) to evaluate patient needs and determine appropriate care management services
- Perform comprehensive assessments to support individualized care planning
- Provide virtual evaluation and management (E/M) services within your scope of practice
- Develop, implement, and oversee care plans that address medical conditions, functional status, and social determinants of health
- Provide clinical oversight and general supervision to care coordinators and advocates in alignment with CMS guidelines
- Collaborate with patients, families, and interdisciplinary team members to improve care continuity and reduce barriers
- Support navigation across healthcare systems, including specialists, services, and community-based resources
- Identify changes in condition and intervene appropriately within scope
- Complete and maintain accurate documentation to support compliant billing for Medicare care management services
- Review and attest to documentation as appropriate for services billed under your NPI

Who You Are
- Active, unrestricted license as a Nurse Practitioner, Physician Assistant, or Physician (Connecticut required)
- Active NPI and PECOS enrollment
- DEA registration (if applicable)
- 2+ years of experience in geriatrics, primary care, chronic disease management, or telehealth
- Strong clinical judgment and ability to manage complex senior patients
- Comfortable working independently in a telehealth environment
- Strong communication and documentation skills
- Familiarity with Medicare documentation and care management services preferred
- Commitment to trauma-informed, patient-centered care
- Maintain active professional liability insurance (minimum $1,000,000 per occurrence / $3,000,000 aggregate) and ability to meet independent contractor coverage requirements
