Job Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of chronic conditions. This role requires a strong commitment to patient-centered care and the ability to work collaboratively within a multidisciplinary team.
As a House Call Nurse Practitioner, you will be responsible for delivering primary care services to patients in their homes. This role requires a compassionate, self-driven NP who enjoys working independently and is committed to improving patient outcomes by providing personalized, high-quality care in a home setting.
Responsibilities
- Prescribe medications and manage chronic conditions
- Order and interpret diagnostic tests (labs, imaging, etc.)
- Provide follow-up care as needed
- Diagnose and treat acute and chronic illnesses, utilizing evidence-based practices.
- Develop individualized care plans in collaboration with patients and their families.
- Perform necessary procedures such as phlebotomy and medical imaging as required.
- Utilize electronic health record systems like eCW for documentation and patient management.
- Provide health coaching to empower patients in managing their health effectively.
- Educate patients on preventive care, healthy lifestyle choices, and management of chronic diseases.
- Maintain accurate patient records in compliance with ICD-9 coding standards.
- Participate in home care visits as needed to ensure continuity of care for patients.
- Collaborate with other healthcare professionals to coordinate comprehensive patient care.
- Adhere to all state and federal healthcare regulations
Qualifications
- Master’s degree in Nursing from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP).
- Valid state nursing license with prescriptive authority.
- Proficiency in electronic health record systems; experience with Cerner or Epic is preferred.
- Strong clinical skills with the ability to perform procedures such as phlebotomy and medical imaging.
- Knowledge of ICD-9 coding and documentation standards.
- Excellent communication skills, both verbal and written, with a focus on patient education.
- Excellent clinical and diagnostic skills
- Ability to work independently as well as part of a collaborative healthcare team.
- Previous experience in home care or health coaching is a plus.
- MUST BE LICENSED IN NYS
- MUST BE CREDENTIALED WITH MEDICAID AND MEDICARE IN NYS
- Fluent in both English and Spanish

Pay: $130,000.00 – $145,000.00 per year
