Remote Patient Care Manager

About the Agency: MedAssist empowers patients facing medical challenges to thrive by addressing their needs with comprehensive access to services through care management. Our Agency is expanding and we are looking to add some great people to our team!

Position Overview: As a Care Manager, you will be providing care coordination for patients who are returning home from nursing homes or rehabilitation facilities. This job entails extensive telehealth communication with patients, therefore, being personable, compassionate, and understanding are important traits. We want to assist our patients with enhancing their health & overall well-being by building lasting connections with them. You will be making phone calls/zoom calls with patients to set up services for them. Must be comfortable speaking on the phone with patients/facilities to coordinate services that assist our patients age in place.

General Responsibilities (including, but not limited to):

  • Researching & coordinating various services such as physical therapy, medical supplies, transportation, etc., in alignment with patients’ needs.
  • Extensively communicate with health providers, patients, and key contacts, to ensure continuity of services.
  • Document patient notes in the electronic health record in an accurate, timely, and detailed manner for approximately 50 patients per month.
  • Follow-up on phone calls, emails, and correspondence promptly to ensure quality standards of service delivery are met.
  • We are able to assist patients in a variety of services! We set up services such as: Home Health Care; Physical Therapy/Occupational Therapy; Transportation; Ordering equipment (a few examples); Wheelchairs; Walkers; Assistive medical equipment.

Requirements and Qualifications:

  • Unrestricted, current nursing license – multi-state compact license and NY licensure

Salary range: $20.00 – $25.00 per hour based upon education/experience and geographic location
