Our LPN Care Specialist position provides Care Coordination to patients with unstable clinical diagnoses and collects patient-centric health information to identify gaps in care, acute risks, and chronic risks. The LPN implements the care plans, routinely monitors the patient’s symptoms and status toward goals, updates interventions and goals based on the patient’s progress, and measures outcomes. Our LPN will set up education modules for chronic diseases as determined in risk tools and refer to licensed educators for the implementation of educational sessions.

Responsibilities:

  • Provides telephonic case management and care coordination
  • Utilizes motivation to change techniques to establish rapport and engage patient
  • Provides culturally competent interface with the patient to secure accurate responses to assessments and to support compliance initiatives
  • Approaches the patient as an advocate, educator, and facilitator of medically necessary healthcare services
  • Promotes patient safety
  • Reviews claims data, review CCDA or EMR, complete risk identification tools to collect data which the RN will use to create a patient-centric care plan and a zone management plan
  • Completes disease-specific risk identification tools and determine appropriate education programs for the patient
  • Monitors monthly from claims and EMR recidivism in ER and inpatient utilization
  • Implements the RN’s patient-centric care plan
  • Promotes timely access to medically necessary and appropriate care
  • Promotes compliance with PCP/Specialist office visits medication
  • Performs medication reconciliation and promote and monitor medication adherence
  • Provides or refers to chronic care education for chronic/complex conditions
  • Monitors and reports outcomes to Care Plan goals and interventions
  • Facilitates patient access to appropriate medical and specialty providers
  • Promotes the integration of physical health and mental health problems and management
  • Assists patients in navigating their healthcare system
  • Promotes quality outcomes and accurate measurement of those outcomes
  • Encourages the medically appropriate use of healthcare resources in the appropriate level of care
  • Accesses and manages PHI in accordance with federal and state requirements (HIPAA, HITEC).
  • Performs other duties as assigned

Qualifications:

  • Three (3) years’ experience minimum in general medicine direct patient care and/or specialty care
  • Preferred two (2) years of Case Management or Disease Management

Certificates, Licenses, Registrations:

  • Current LPN licensure in at least one state

Education and/or Experience:

  • A minimum of three years of direct patient care
  • Telephonic or face-to-face care coordination/case management preferred

Computer Skills:

  • Microsoft Office: Outlook, Word, and Excel

Job Type: Full-time

Pay: $38,000.00 – $42,000.00 per year
