Full Job Description
Fast-growing Remote Telehealth Company is looking for LPNs that can work remotely with patients with chronic conditions such as hypertension, diabetes, and obesity.
Prior Homecare Experience Required.
Our nurses work over the phone and via Zoom. Their primary responsibilities are to provide Education, Motivation, and Coaching that help patients improve their chronic conditions.
Our nurses excel at establishing rapport, and assisting patients overcome health related obstacles in their daily lives. This includes Nutritional Counseling and Diabetes Education.
