Our healthcare workers are experiencing a crisis never before seen in this lifetime. At Upward Health, we have a unique opportunity for front-line staff to continue to care for the most high-risk, high-need patients in a model that allows them to do it in a way that minimizes their personal risk. Our core business is as a home-based medical group specializing in primary medical and behavioral care for individuals with complex needs. We serve patients throughout their communities, and we diagnose, treat and prescribe anywhere our patients call home. Beyond medical supports, we also assist our patients with challenges that may affect their health, such as food insecurity, social isolation, housing needs, transportation and more. Much of our care is delivered to our patients in their home through Facilitated Virtual Care, our unique approach to telemedicine.

During this COVID-19 Crisis, our environment is one where NPs can work directly with patients providing much needed ongoing care for their current medical and behavioral health needs, which also playing a critical role in the fight against COVID-19. We offer a screening and testing service that allows our patients who are the most vulnerable under the best of circumstances to receive screening, testing and treatment for COVID-19 in the comfort of their own homes, and your role would be to facilitate that process for our patients. As an employer, our mission is to take care of our employees so that they can take care of our patients. As an NP for Upward Health, you would have the opportunity to provide the care you are so dedicated to provide while keeping yourself and your family safe from exposure, all while given the proper tools and equipment to deliver that care safely and effectively.

We are currently in need of Nurse Practitioners who are actively licensed, preferably in Louisiana and Rhode Island, although any active state license is acceptable at the present time. Our ideal NPs are individuals who are motivated to provide high quality, patient-centered, comprehensive primary care in a telemedicine setting, triaging patients through phone calls and video chats, with the potential for an occasional field visit to deliver direct care only if medically necessary.
