Role DescriptionThis is a full-time remote role for a Nurse Practitioner. The Nurse Practitioner will be responsible for providing direct patient care, including assessing, diagnosing, planning, and implementing individualized patient care plans. They will manage and treat acute and chronic conditions, prescribe medications, and collaborate with interdisciplinary teams to ensure optimal patient outcomes. Patient and family education, counseling, and support are also critical components of this role. This NP will see patients in Central Indiana.

Qualifications

• Advanced clinical skills in assessing, diagnosing, and managing acute and chronic health conditions.
• Experience in patient and family education, counseling, and support.
• Strong communication and collaboration skills to work effectively within interdisciplinary teams.
• Proficiency in prescribing medications and managing treatment plans.
• Empathy, compassion, and a deep commitment to providing high-quality palliative care.
• Active Nurse Practitioner license
• certification in palliative care (not required at this time)
• Master’s degree in Nursing (MSN)
• experience with in home care preferred
