Role Description
There are multiple open positions including Onsite Full Time and PRN Float roles for a Nurse Practitioner at One to One Health. The Nurse Practitioner will be responsible for providing primary and acute care, managing chronic diseases, conducting biometric screenings, offering health coaching, and delivering preventative care. The role involves collaborating with a multidisciplinary team to ensure the well-being of patients across various healthcare settings.
Qualifications
- Primary and Acute Care skills
- Chronic Disease Management and Preventative Care experience
- Biometric Screenings and Health Coaching proficiency
- Occupational Therapy knowledge
- Strong communication and collaboration skills
- Ability to work independently and remotely
- Experience in data analytics and radiology is a plus
- Active Nurse Practitioner license and certification
