How We Serve
The Nurse Practitioner (NP) is a key clinical role providing leadership for a team of 5-7 colleagues to deliver exceptional care to patients. This position works in concert with RNs, LPNs, RTs, and CMAs that meet directly with the patient in their home and will develop trusting relationships with the patients. The NP works with the Clinical Care Team via video, phone, or text communications to further support and direct their work with patients. The NP is accountable for clinical care in the assigned population and the leadership of the team.
Members of the Clinical Care Team will meet directly with patients in the patient’s home. The NP works with the clinicians to understand the patient’s unique needs. Each patient is assessed through an intake process to identify and define a plan of care. Based on the situation, the NP may diagnose and recommend a clinical path of action, including ordering labs, imaging, and/or treatments as required. This role will regularly communicate with PCPs and specialists involved in the patient’s care to encourage a single, coordinated plan for the patient.
This individual will primarily work independently under physician oversight. This role will utilize support from others on the Spiras Health team as needed to ensure only high-quality care is delivered to their patients. The role will ensure the clinical team follows defined standards, protocols, and processes to document and manage interactions with patients and other providers. The right candidate will have an interest in providing leadership and professional development to a small clinical team. Additionally, this role will provide clinical expertise through virtual care delivery working with the colleagues in the field who are the eyes, ears, and hands of the Clinical Care Team. A typical day for one of our nurse practitioners may include:
- Working directly with the 5-7 colleagues to support their patient panel
- Leading Clinical Team Coach to ensure protocols are followed
- Reviewing medical records
- Working with patients directly via video, phone, secure communications
- Answering clinical questions, assessing patients, ordering labs, DME, and medications
- Reviewing patient panels to address gaps or questions about the panel
- Talking with other PCPs and specialists regarding a patient
- Assisting with patient advocacy and addressing barriers or gaps with care
- Managing an exacerbation event in collaboration with Clinical Care Team
- Documenting all interactions in the Spiras EMR
Duties and Responsibilities
- Review charts against defined protocol for consistent care delivery
- Collaborate with providers to ensure care coordination
- Order clinically appropriate services such as imaging, DME, and medications
- Manage emergent after-hours care needs to patients per scheduled call
- Collaborate with other Spiras clinicians in service to the patient
- Navigate EMR platform to document patient interactions
- Document clinical and quality information consistently and concisely
- Ensure compliance with HIPAA, company policies, and state and federal guidelines
- Perform other duties as assigned
Qualifications
- Board-certified nurse practitioner in good standing
- 7+ years of experience with adult / geriatric population
- Demonstrated experience working with multiple chronic conditions such as COPD, CHF, Diabetes, Asthma, etc.
- Active licensure in state of residence and willingness to pursue additional states
- Medicare Billing ID
Skills Required:
- Experience with cardiac or pulmonology specialties
- Experience coordinating care with health plans and health systems
- Experience with advance directives and palliative care
- Strong and concise communicator able to rapidly identify and gather required information to assess and treat patients
- Comfortable working with patients that may have difficulty hearing, struggle with memory issues, or have a lower health care literacy level
- Willingness to collaborate with others
- Ability to think quickly and provide information with confidence and clarity
- Ability to manage competing priorities, organize work efficiently, and meet deadlines
- Perseverance and self-management to achieve defined objectives
