About the Role

We are seeking an experienced and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join a growing virtual heart failure and cardiology care team. This telehealth-based position partners with outpatient cardiology practices across Florida to deliver advanced, patient-centered heart failure care through a proprietary telehealth platform.

Providers join patients virtually via our secure app, supporting guideline-directed therapy, patient monitoring, and remote management of heart failure and related cardiovascular conditions. The position begins part-time (20 hours per week) as we expand into new partner markets, with a planned ramp-up to full-time (40 hours/week) as patient enrollment increases.

Some on-call responsibilities will be required to cover urgent remote monitoring alerts or patient escalations.

Key Responsibilities

  • Provide virtual management of heart failure patients in collaboration with partnering outpatient cardiology groups.
  • Conduct telehealth visits using our integrated digital health platform and connected remote monitoring tools.
  • Initiate, titrate, and optimize guideline-directed medical therapy (GDMT) for heart failure patients.
  • Assess and manage volume status and respond to weight changes, blood pressure variations, or other clinical alerts.
  • Educate patients and caregivers on disease management, medications, and symptom awareness.
  • Collaborate closely with cardiologists, pharmacists, RNs, and care coordinators to ensure seamless communication and continuity of care.
  • Participate in on-call coverage to manage urgent patient needs or monitoring alerts after hours.
  • Maintain accurate and timely documentation within electronic medical records (EMR).
  • Contribute to ongoing quality improvement initiatives, including metrics such as hospital readmission reduction and GDMT adherence.

Required Qualifications

  • Active and unrestricted NP or PA license in Florida.
  • Board certification as NP or PA.
  • Heart failure experience or strong background in emergency medicine managing cardiovascular and decompensated HF patients.
  • Experience with telehealth or virtual care platforms.
  • Proficiency in interpreting lab results, imaging, and device data remotely.
  • Strong clinical judgment, with ability to manage complex cardiac patients without in-person exams.
  • Excellent communication and collaboration skills.
  • Willingness to participate in an on-call rotation for remote patient monitoring alerts.

Preferred Qualifications

  • Master’s or Doctoral degree in Nursing or Physician Assistant Studies.
  • Experience partnering with outpatient cardiology or heart failure programs.
  • Familiarity with GDMT titration protocols and remote monitoring data workflows.
  • Prior work with value-based care or population health metrics.

Schedule & Growth Opportunity

  • On-Call: Limited after-hours coverage rotation to support patient alerts or urgent telehealth needs.
