Your Role
Nurse Practitioners are at the heart of the Firefly clinical team, providing extraordinary virtual first primary care with expert clinical skills and an empathic approach. They help us deliver industry-leading outcomes by managing patients as a team with our Primary Care Physicians, Behavioral Health Specialists, and Health Guides.

What you’ll do:
- New patient onboarding
- Conduct initial visits with Firefly patients via video visit
- Create longitudinal care plans by working with each patient to generate a list of goals and associated tasks
- Chronic disease management
- Develop care plans for specific conditions (eg, hypertension, diabetes) to help patients manage and control their chronic disease
- Collaborate with Health Guides, who will conduct regular video visits to help patients progress toward goals
- Liaise with the behavioral health team to triage, diagnose, and treat mental illness that is appropriate for primary care setting
- Work across disciplines with a network of virtual specialists to diagnose and manage more complex illness
- Addressing acute complaints
- Support RN staff in triaging and addressing new complaints that arise from patients via Firefly mobile application or phone calls
- Manage care plans to incorporate new issues and ensure follow-up
- Prevention and wellness
- Create wellness and prevention goals with patients, using standard templates for prevention (eg. cancer screenings, immunizations)
- Develop and implement tools to help patients manage wellness goals
You’ll be a good fit if you:
- Are a graduate of state-accredited nurse practitioner Have board certification: AANP, ANCC
- Have current licensure to practice as a nurse practitioner in Massachusetts and at least one of the following states: NY, NJ, MD, WV, VA, VT, TN, SC, RI, PA, NC, DE, CT
- Have a current federal DEA registration and Massachusetts Controlled Substance Registration
- Have the ability to work 11-7 EST shift
- Have experience working effectively on teams to care for patients
- Have excellent written and verbal communication skills
- Have at least two years of experience in a primary care setting
- Have experience in telemedicine (a plus)
- Speak Spanish (a plus)
- Have experience with tools related to process improvement (a plus)
Our office is located in Watertown, Massachusetts, and while this role can be done largely remotely, there are several times of year when staff come together onsite for planning and team building. Accordingly, we do require clinical staff to be fully vaccinated against COVID.
