Position Summary:
As a Remote Nurse Practitioner specializing in Virtual Care Management (VCM), you will deliver comprehensive healthcare services to patients via telehealth platforms. You will assess, diagnose, and treat a wide range of medical conditions, collaborate with physicians, and help manage chronic conditions, ensuring that patients receive the best care in a remote setting. The role involves evaluating health status through virtual visits, educating patients, and providing guidance on treatment plans and wellness strategies.
Key Responsibilities:
- Remote Patient Assessments: Conduct virtual consultations with patients using telemedicine platforms, obtaining medical history, performing physical assessments via video, and discussing symptoms.
- Diagnosis & Treatment: Diagnose acute and chronic conditions, prescribe medications, and provide treatment recommendations.
- Care Plan Development: Create, monitor, and adjust individualized care plans for patients based on ongoing assessments.
- Health Education & Support: Educate patients on managing chronic conditions, preventive care, and health promotion strategies through virtual consultations and messaging.
- Patient Monitoring: Monitor patient progress, follow up on care plans, and adjust treatments as necessary. Utilize remote patient monitoring tools to track patient data (e.g., blood pressure, glucose levels, etc.).
- Collaboration: Work closely with a multidisciplinary team of healthcare providers, including physicians, specialists, and care coordinators, to ensure integrated care and positive outcomes for patients.
- Documentation: Maintain accurate, timely, and thorough documentation of patient interactions, diagnoses, treatments, and follow-up plans in the electronic health record (EHR) system.
- Technology Proficiency: Utilize remote care platforms, EHR systems, and digital communication tools to deliver services efficiently and securely.
- Compliance & Regulatory Adherence: Ensure all services are provided in compliance with relevant healthcare laws and regulations (e.g., HIPAA, state licensure requirements).
Qualifications:
- Education: Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP).
- Licensing: Active Nurse Practitioner (NP) licensure with eligibility for licensure in the state(s) where patients are located.
- Certifications: Board certification as a Nurse Practitioner (Family NP, Adult-Gerontology NP, or relevant specialty depending on the patient population).
- Experience: Previous experience in a healthcare setting, ideally with experience in telemedicine, virtual care management, or a related field.
- Skills:
- Strong clinical assessment and diagnostic skills.
- Excellent verbal and written communication abilities for remote patient care.
- Ability to manage and coordinate patient care virtually.
- Proficient in the use of telehealth platforms, EHR systems, and digital tools.
- Ability to work independently and as part of a virtual care team.
Preferred Qualifications:
- Experience in a Virtual Care Management or telehealth role.
- Knowledge of chronic disease management and telehealth best practices.
- Experience with remote patient monitoring technology.
