Job Description:
Job Summary: We are seeking a highly motivated and experienced Nurse Navigator (NP/RN) with a strong background in clinical operations to join our team. This individual will play a critical role in designing and developing the conversational tree for Carrie, workflow implementation and define the strategy of the product as the company scales. The ideal candidate will have extensive experience in patient care, clinical operations, and care coordination, with a passion for leveraging technology to enhance patient outcomes.
Key Responsibilities:
· Conversational Tree Design: Collaborate with our product development team to design and implement the conversational tree for Carrie. This will involve mapping outpatient interactions, decision pathways, and clinical scenarios that Carrie will handle over time.
· Clinical Content Development: Provide expert input on the clinical content and protocols that Carrie will use, ensuring accuracy, relevance, and compliance with healthcare standards.
· Care Coordination: Utilize your experience in care coordination to ensure that Carrie’s conversations align with best practices in patient navigation and chronic disease management.
· Patient Engagement Strategies: Develop strategies to enhance patient engagement and adherence through Carrie’s interactions, focusing on empathy, clarity, and patient-centered care.
· Testing and Iteration: Participate in the testing and iteration of Carrie’s conversational abilities, providing feedback based on clinical knowledge and patient care experience.
· Cross-Functional Collaboration: Work closely with engineers, designers, and other stakeholders to integrate clinical insights into the product development process.
· Training and Documentation: Assist in creating training materials and documentation to guide the use of Carrie by healthcare providers and patients.
Qualifications:
· Licensure: Current licensure as a Nurse Practitioner (NP) or Registered Nurse (RN).
· Experience: Minimum of 5 years of clinical experience, strong command of CHF and chronic comorbidities, including experience in care coordination or patient navigation.
· Clinical Operations: Proven experience in clinical operations, particularly in managing chronic diseases especially heart failure, diabetes, or COPD.
· Patient Interaction: Strong background in patient education, engagement, and support.
· Technical Skills: Familiarity with digital health platforms, EHR/EMR systems, and patient management software. Experience with AI or conversational interfaces is a plus.
· Communication: Excellent communication skills, both written and verbal, with the ability to convey clinical information clearly and effectively.
· Collaboration: Ability to work effectively in a cross-functional team environment, collaborating with engineers, designers, and other healthcare professionals.
· Problem-Solving: Strong analytical and problem-solving skills, with a proactive approach to addressing challenges in product development.
· Patient-Centered Mindset: A deep commitment to improving patient outcomes and enhancing the patient experience through innovative healthcare solutions.
Preferred Qualifications:
· Specialization: Experience in cardiology, particularly in managing patients with heart failure.
· Project Management: Experience in project management within a healthcare setting.
· Technology Integration: Experience in integrating clinical workflows with technology solutions.
