Job Summary: As a Remote Certified Medical Assistant or LPN, you will play a crucial role in engaging chronically ill patients through monthly phone consultations, guiding them on best practices for self-management. You will utilize electronic medical records and data from devices like blood pressure cuffs and weight scales, ensuring seamless communication with physicians to improve patient care continuity.

Key Responsibilities:

  • Document all time spent coordinating care. Achieve a minimum of 90% efficiency.
  • Collaborate with the Nurse Manager to strategize patient engagement.
  • Achieve realistic productivity goals while maintaining high-quality care.
  • Document patient interactions and findings accurately.
  • Escalate any real-time or emerging health events appropriately.
  • Encourage patients to actively use electronic devices for condition monitoring.
  • Build lasting relationships through ongoing verbal communication and support.
  • Utilize the Healthmote platform to document patient interactions.
  • Uphold patient confidentiality, rights, and dignity in all interactions.
  • Employ persuasive coaching techniques to motivate patients in self-management.
  • Perform additional duties as required.

Qualifications:

  • Completion of an approved Medical Assistant training course or LPN license.
  • Basic knowledge of medical terminology and vital signs.
  • Proficient in using a personal computer for documentation and medical history retrieval.
  • Excellent communication and customer service skills.
  • Ability to work independently with minimal supervision.
  • Bilingual communication skills are helpful
  • Preferred: Experience in Chronic Care Management, transitional care management, remote patient monitoring

Experience Required:

  • Minimum of 2 years of experience as a Medical Assistant or 1 year as LPN.
  • High school diploma or GED (preferred but not required).
  • Experience in a medical environment is highly desirable.

Work Schedule:

  • This position requires a commitment to a 40-hour work week. Typically, 9-5:30 eastern standard time. Monday to Friday.

Starting Pay:

Medical Assistants :$16 to $19 based on experience

LPN’s: $18 to $22 based on experience

Includes: Paid time off and holiday pay

  • This is a work-from-home position. We do not supply equipment. You must have a computer with at least 8 gigs of RAM, and reliable access to the internet to use our software. Must have a headset, or obtain one.

Job Type: Full-time

Pay: $16.00 – $22.00 per hour
