Quality Health Partners supports primary care physicians achieve 5-star quality ratings by placing a clinical quality team in provider practices or via telemedicine for the sole purpose of taking care of the administrative, documentation, and coding requirements to close quality gaps in care, capture Hierarchical Condition Categories (HCCs), completing the Annual Wellness Visit/Annual Health Assessments and, Well Child Visits, for their patients.
Physician Assistant Job Responsibilities:
- Perform Annual Health Assessments/Annual Wellness Visits, Well Child Visits
- Promotes and maintains patient health by providing preventive medical services.
- Contributes to a physician’s efficiency by identifying short-term and long-range patient care issues.
- Assesses patient health by interviewing patients and performing physical examinations including obtaining, updating, and studying medical histories.
- Provides anticipatory guidance and developmental screenings for children.
- Assesses and documents chronic conditions to the highest degree of specificity.
- Documents patient care services by charting in patient Electronic Medical Record (EMR).
- Provides assessment and plan for each diagnosis.
- Maintains a safe and clean working environment by complying with clinic procedures, rules, and regulations.
- Protects patients and employees by adhering to infection-control policies and protocols. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; and enforcing adherence to requirements and advising management on needed actions.
- Complete charts by signing and locking after coding review.
- Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

In this position you will not prescribe medications, change medications, and will not perform procedures. These patients have a primary care physician (PCP) who manages their healthcare needs. You will not provide follow-up services. Any acute issue will be referred back to the PCP. The services that QHP provides are done on an annual basis for the purpose of documenting and assessing chronic conditions and to close quality care gaps.
Education and Experience Requirements:
- Graduate from an ARC-PA accredited physician assistant program
- State license
- BLS certification
- National Provider Identifier (NPI)

Salary: $50,000.00 – $141,422.98 per year
