What You’ll Do

What You Get to Do:

• You will leverage your primary care and geriatric expertise to provide virtual, patient-centered, whole-person, team-based care for a panel of adults 65+.
• You will collaborate with an internal care team made up of physicians, nurses, health champions, Patient Service Coordinators, and Behavioral Health Specialists to deliver a best-in-class healthcare experience for patients.
• You will foster patient and family leadership, integrate geriatric principles, assertively co-manage with specialists and hospitalists, and practice shared decision-making.
• You will gather patient histories, conduct physical exams, diagnose conditions including geriatric syndromes, order appropriate diagnostic tests, interpret results, as well as develop and implement treatment plans given the patient’s goals of care and current condition.
• You will conduct virtual care, offering access in several ways including virtual visits (video or phone), secure messaging, and hybrid co-visits where you will be present on video while a registered nurse conducts assessments in person in the home.
• You will work with Patina’s Electronic Medical Record (EMR) and separate care team application to efficiently and thoroughly document diagnoses, create proactive longitudinal care plans, and coordinate with your team.
• You will participate in daily team huddles and weekly interdisciplinary rounds to seamlessly collaborate on care for Patina patients.
• You will participate in a rotation of phone-based on-call coverage for after-hours support of Patina patients.
• You will be a positive force for change and growth within the care team, providing empathetic and direct feedback to teammates, integrating the feedback you receive, and helping to identify, prioritize, and pilot opportunities for continuous system improvement.
• You will aid in implementing and refining a system of patient care delivery that encourages communication, teamwork, positive outcomes, high quality care, patient satisfaction, and proper application of resources in order to inspire each older adult to take charge of their life and health.

What Is Required

What is Required:

• 3+ years of experience providing primary care to the 65+ population
• An active unrestricted Nurse Practitioner license or ability to obtain one in PA, NC, SC and future states as Patina grows.
• National board certification as a Nurse Practitioner from an accredited agency.
• Possession of DEA registration or eligibility required.
• Participation in continuing education as required by the Board of Nursing.
• Ability to work from home in a HIPAA compliant location.
• Ability to work on-call rotation.
• Passionate about caring for adults 65+ and providing whole-person, patient-centered care.
• Enthusiasm about being part of a growing start-up company, the ability to deal with ambiguity and balance multiple priorities, learning agility, and a willingness to be flexible in your role as processes and needs change.
• Highly capable navigating healthcare technology platforms seamlessly while providing an exceptional patient experience, using different technical modalities interchangeably (eg. computer, tablet, phone), and learning new technologies.
• Strong team collaborator with proven ability to effectively communicate in writing and verbally.

What Would be Great to Have:

• Prior experience with providing home-based and virtual (strongly preferred) care
• Certified in adult-gerontology as a nurse practitioner
• Experience in start-up and/or fast growing and dynamic organization
• Relationships and understanding of the Philadelphia and Charlotte areas

Location:

Remote. Charlotte or Philadelphia areas preferred. Must be willing and able to work Eastern Standard Time zone hours (8 am – 5pm ET , as well as on-call rotation).

At Patina, our salary bands are determined by the role and career level. The range displayed on each role posting reflects the minimum and maximum target salary (total cash compensation) for the position across all locations. Within the band, individual pay is determined based on skills, experience, capabilities, work location, required licensure and certifications. Furthermore, based on our compensation philosophy, which is grounded in hiring mission-driven and talented crew members and paying competitively while ensuring pay isn’t their sole determinant for joining or staying at Patina, we generally target the middle of the range for our total cash compensation package.

The total cash compensation band for this position is $113,000 – $165,000 + benefits.
