We have an opening for a PRN RN COVID Care Navigator to work with our National Virtual Health clients on a remote basis!

Essential Functions:

RN responsibilities:

  • Establishes supportive relationship with member
  • Provides COVID-19 condition specific self-care education to member enrolled in the Care Navigation program
  • Assesses member’s educational/clinical needs, prioritizing and individualizing program based on clinical symptoms and elicited information
  • Provide member education effectively by phone
  • Acts as resource for members
  • Documents sessions and progress into the electronic medical record
  • Collaborates with the National Virtual Health providers or local community care to arrange for further medical treatment when needed
  • Consults with National Virtual Health providers to solve problems and seek advice
  • Provide education and interventions with and for the member to improve outcomes
  • Identifies emergency situations and provides recommendations within legal scope of practice
  • Maintains member privacy and confidential member information
  • Participates in outreach/follow-up phone calls and follow-up member visits
  • Assists in the development of criteria to measure outcomes, assists in the analysis of data for quality improvement, interprets study results, identifies limitations and barriers and recommends alternatives
  • Serves as member advocate
  • May require other duties as assigned

Basic Qualifications


  • Certificate from an accredited Nursing School or Associate degree (A.S.) required; Bachelor’s degree (BSN) from four-year college or university preferred
  • Currently licensed Registered Nurse in state of practice
  • Current certification AHA or ARC Basic Life Support for health care providers


  • At least three years’ experience as a Registered Nurse
  • Experience caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension. (preferred)
  • Knowledge and experience with Electronic Medical Records (preferred)

Knowledge and Skills:

  • Knowledge of Electronic Health Records, internet software, spreadsheet software and Word Processing software
