Our Clinicians provide evidence-based interventions via the Collaborative Care Model to promote symptom reduction and optimal patient outcomes. You are a core member of a Collaborative Care team that involves the patient’s OBGYN and /or Primary Care Provider, Psychiatric Consultant, and other Behavioral Health providers.

Schedule:
- Part-time: 3-5 hours async document review per week requiring daily remote admin
- Conduct Rx management patient visits
- One Caseload Review session per week led by our Medical Director / Psychiatrist
- Opportunity to expand your role over time (ability to create your own schedule)
Salary: $90-115 per hour
Job Description:
We are seeking a licensed PMHNP with an active license in Connecticut with a passion for women’s wellness to join our integrated care model team.
Responsibilities:
- Provide psychiatric consultation to OBGYN and primary care providers through case systematic case review with a panel of Collaborative Care Clinicians.
- The majority of your work will be in consultation with primary care providers and our team of clinicians. From time-to-time you may be required to meet with patients.
- Provides regularly scheduled (usually weekly) video caseload consultation to Concert Health Collaborative Care Clinicians, focused primarily on high-risk patients, patients new to treatment, or those not improving as expected.
- Suggests treatment plan changes, including medication recommendations and behavioral treatment, for patients
- Educates OBGYN and primary care providers and Collaborative Care Clinicians about mental health/substance abuse disorders/treatment options
- Documents treatment recommendations in the electronic health record and communicates with primary care provider as requested
- Collaborates with assigned Collaborative Care Clinicians to track and oversee patient panels and clinical outcomes using our cloud-based patient registry
- Communicates periodically with Collaborative Care Clinicians/OBGYN/PCPs for urgent matters outside weekly check-in
Qualifications:
- Required: PMHNP licensed in Connecticut – in good standing
- Comfortable working remotely in a virtual setting
- Experience in “treat to target” models and Collaborative Care model
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation
- Knowledge of/experience with behavioral health tools (PHQ9, GAD7, CSSRS, etc.)
- Experience practicing/collaborating in primary care or integrated care settings
- Tech-savvy and ability to navigate multiple partner EHRs and systems.
