One hundred FDA-registered and approved devices are available to support patients at home with full diagnostic monitoring and support, aiming to prevent re-hospitalizations and health exacerbations.
JOB OVERVIEW:
Circadian Health is currently seeking an experienced, COPD Care Manager to join our rapidly evolving Tele-Pulmonary services team. New York state license is required.
Job Summary:
- The day-to-day role focuses on education, symptom monitoring, and care coordination.
- Clinical Assessments: Conducting comprehensive telephonic interviews to evaluate a patient’s respiratory status, including shortness of breath (dyspnea), cough severity, and oxygen saturation levels.
- Reviewing and triaging physiologic data from remotely connected devices
- Disease Education: Teaching patients and caregivers about COPD pathology, the importance of smoking cessation, and proper nutrition.
- Medication Management: Verifying adherence to maintenance inhalers and ensuring the patient knows how to use “rescue” medications during a flare-up.
- Action Planning: Developing “COPD Action Plans” that help patients identify “Green” (doing well), “Yellow” (caution/flare-up starting), and “Red” (emergency) zones.
- Care Coordination: Acting as a bridge between the patient, their primary care physician (PCP), pulmonologists, and home health services like oxygen providers.
- Social Determinants of Health (SDOH): Identifying barriers to care, such as lack of transportation to appointments or inability to afford expensive inhalers, and connecting patients with community resources.
- Communicate with supervising NPs/MDs through established regular channels of communication, including scheduled meetings, phone consultations, and electronic communication (email, secure messaging, internal messaging platform).
Job Specifications:
Strong communication skills, collegial team dynamics with an emphasis on quality, patient- focused care are key characteristics. Previous experience and a strong background in Pulmonology are preferred.
Requirement
- Education: Associate or Bachelor of Science in Nursing is required
- Licensure: Active Registered Nurse (RN) or (LPN) license; New York state license is required; a Compact License for multi-state telephonic work preferred
- Experience : 3–5 years of clinical experience in acute care, care management, home health, or pulmonology. Knowledge of COPD remote monitoring: Spirometry, Peak Flow, pulse oximetry technologies preferred. Experience in initiating and modifying COPD treatment plans. Knowledge and experience with COPD medications and therapies
- Technical Skills: Proficiency in Electronic Medical Records (EMR), Google Business Suite, Prior Authorization platforms, RPM device troubleshooting
- Soft Skills: High empathy, active listening, and the ability to build trust without face-to-face contact. Passion for improving patient outcomes long-term
Additional qualifications:
- Spanish language skills preferred

- Full-Time, Remote Monday – Friday: typical business hours

Pay: $35.00 – $45.00 per hour
