Job Title: RN Case Manager – Disease Management
Pay: $39 – 41/hr (Based on experience, location, etc.)
Location: 100% Remote – Must be able to support PST, MST, and CST time zones.
Job Type: Temp. to hire (conversion is based on performance, and typically takes place around the 4th or 5th month of assignment)
Work Schedule: Full-time, 40 hours/weekly; Days – Monday to Friday

Job Summary: Seeking an experienced Registered Nurse (RN) to provide Disease Management (DM) services. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patients’ health and psychosocial needs to empower them in managing chronic conditions. The Disease Management Nurse will collaborate with an interdisciplinary care team to identify appropriate interventions, prevent complications, and minimize the risk of exacerbation using evidence-based, condition-specific education and coaching.
This position requires the ability to develop individualized care plans in partnership with providers, patients, and families, while continuously assessing the effectiveness of interventions to improve health outcomes.
DOD Credit Check: This position does involve a DOD background check, primarily used to identify significant financial concerns, such as patterns of financial irresponsibility, excessive debt, or ongoing disputes with multiple creditors. Financial issues alone do not automatically disqualify a candidate from DoD positions. However, applicants are expected to provide a reasonable explanation and demonstrate that they have taken appropriate steps to address and manage those concerns responsibly.
Essential Requirements:
- Must have an RN Compact License within the state of practice
- Must be able to pass a Department of Defense (DoD) background investigation.
- Minimum of three years of clinical experience in a healthcare setting.
- Experience in case management, care coordination, or discharge planning in an acute, ambulatory, or specialty disease management program within a healthcare system or managed care organization (MCO).
- Preferred – Previous experience in Disease Management is highly preferred. If not, candidates with a background in behavioral health and chronic disease management will also be considered.
- Preferred – Bachelor’s degree in nursing or a related health field.
- Preferred – Knowledge of government healthcare programs, including TRICARE, VA, Medicare, and Medicaid.
