What You’ll Do:

Provide patient-centered virtual care focused on chronic disease management, transitional care, and preventive health.

Evaluate, diagnose, and treat adult and senior patients remotely.

Coordinate follow-up care, especially after hospital discharge, to help reduce readmissions.

Document visits via EMR and use telehealth tools to deliver effective, empathetic care.

Collaborate with a diverse, mission-driven team that includes social workers, medical providers, and care coordinators.

What You’ll Bring:

California license as an ARNP or PA

National certification (NP or PA-C)

2+ years of experience in primary care, geriatrics, or chronic disease management

Experience with telehealth and vulnerable populations is highly preferred

Bilingual (English/Spanish) strongly preferred

A passion for equity in healthcare and comfort with virtual care platforms
