The Qualified Healthcare Provider (QHP) will play a crucial role in conducting telehealth Qualifying Visits (QV) for patients with Obstructive Sleep Apnea (OSA) and providing ongoing support to Respiratory Therapists (RTs) and Care Navigators. The QHP will be responsible for the initial assessment of patients, reviewing their medical history and OSA diagnosis, and ensuring appropriate enrollment in Chronic Care Management (CCM) and Remote Physiologic Monitoring (RPM) services. Following the QV, the QHP will serve as a clinical resource for RTs and Care Navigators, offering guidance and expertise as needed. The QHP will work under the supervision of a physician Medical Director who will monitor their work and provide additional clinical supervision and consultation.

Responsibilities:

  • Conduct comprehensive telehealth Qualifying Visits (QV) with each patient, reviewing their medical history, OSA diagnosis, and current treatment plan. Document the QV thoroughly on a CEHRT EHR platform.
  • Assess patient eligibility and facilitate enrollment in Chronic Care Management (CCM) and Remote Physiologic Monitoring (RPM) services.
  • Collaborate with Respiratory Therapists (RTs) to develop personalized care plans for patients, ensuring optimal CPAP therapy settings and adherence to treatment protocols.
  • Provide ongoing support and consultation to RTs, addressing questions, interpreting complex cases, and recommending treatment adjustments as needed.
  • Collaborate with and supervise Care Navigators who interact with patients on additional chronic conditions, providing guidance and clinical oversight.
  • Review patient data collected by RTs and Care Navigators, including CPAP usage data, patient-reported outcomes, and information related to other chronic conditions, to monitor progress and identify potential issues.
  • Conduct follow-up telehealth visits with patients as necessary, based on referrals from RTs or Care Navigators or to address specific concerns.
  • Stay informed about the latest advancements in OSA treatment and technology to ensure evidence-based care.
  • Work closely with the physician Medical Director, seeking guidance and consultation on complex cases and clinical decision-making.

Qualifications:

  • Active and unrestricted California license as a Physician Assistant (PA)
  • Additional licensure in multiple states is highly desirable.
  • Demonstrated knowledge and/or experience in managing patients with OSA, including familiarity with CPAP therapy and interpretation of sleep data.
  • Proficiency in telehealth platforms and technology (Zoom, etc.).
  • Excellent communication and interpersonal skills to foster effective collaboration with RTs, Care Navigators, and patients.
  • Strong analytical and problem-solving abilities to assess patient data and recommend appropriate interventions.

Schedule:

  • Primarily scheduled visits between 9 am and 5 pm Pacific Time.
  • Flexibility to conduct ad-hoc visits during extended business hours, including evenings and weekends, to accommodate patient needs and support RTs and Care Navigators.

Job Type: Contract

Pay: $85.00 – $110.00 per hour

Expected hours: 10 – 40 per week
