About the role:
As a Virtual Advanced Practice Clinician, you will be a part of a pilot program in our model to care for patients via virtual modalities in partnership with our field-based community health workers and our chronic disease management, behavioral health, and pharmacy teams. As part of our team-based practice, you will collaborate with patients’ existing primary care and specialist providers by extending their care to fill any gaps in medical, behavioral health, or social needs and ensure our patients receive best-in-class, evidence-based, and patient-centered holistic care.

Responsibilities will include:

  • Performing facilitated and non-facilitated virtual initial patient visits and annual comprehensive visits to obtain a complete biopsychosocial view of patients’ health with a focus on thoughtful, accurate, and specific documentation and addressing quality gaps in care
  • Ordering and following up on appropriate laboratory or imaging tests to aid in evidence-based diagnosis, preventative care, and creation of treatment plans
  • Applying clinical judgment in concordance with Accompany Health clinical guidelines to provide accurate and appropriate clinical diagnosis and guidance on treatment plans
  • Referring patients into appropriate chronic disease management, behavioral health, or medication adherence programs as needed and collaborating with our RN, BH and pharmacy teams on appropriate treatment plans
  • Collaborating and communicating with external providers and community-based organizations to advocate for care aligned with best-in-class evidence-based care and patient goals
  • Participating in rotating call schedule to address patients’ urgent clinical needs and support ways to increase healthy days at home
  • Ensuring timely and appropriate documentation within our electronic medical record as well as our custom-built platforms that support care for our patients
  • Providing feedback and engaging in rapid iteration / adaptation of workflows as part of pilot team

Desired skills and experience:

  • Required
  • Active Nurse Practitioner or Physician’s Assistant license in the state of Michigan
  • AANC or AANP board certification for Nurse Practitioners OR NCCPA board certification for Physician Assistants
  • Active DEA Controlled Substance Registration or eligible for application. X-waiver licensure (for buprenorphine prescribing) is a plus but not required
  • Active controlled substance license in state in which you are seeking employment or eligible for application
  • Certified in Basic Life Support for Healthcare Providers
  • 2+ years of clinical experience in providing primary care in an internal medicine, family medicine, or geriatric medicine setting
  • Proven skills, knowledge base, and judgment necessary for independent clinical assessment and decision-making of acute and non-acute clinical problems
  • Comfort and experience in working within an electronic medical record
  • Valid unrestricted driver’s license and access to an insured vehicle for daily use

  • Preferred
  • Experience in adult internal medicine, family medicine, and/or geriatrics
  • Bilingual proficiency in Spanish or Arabic is strongly preferred
  • Multi-state licensure or willingness to obtain additional licensure as requested
  • Flexible to work Monday-Friday 8:00am-5:00pm with willingness to work 10:00am-7:00pm or 11am-8:00pm ~2 days per month/please note that schedule may be adjusted to accommodate patient care and operational needs
  • Experience in value-based care organizations, including accurate and appropriate clinical documentation and quality HEDIS measures
  • Experience and passion for caring for individuals with complex chronic medical, behavioral and social needs
  • Experience with engaging individuals with untreated and/or symptomatic chronic mental illness and addiction
  • Prior experience working as part of a multidisciplinary care team

To keep our patients, communities and each other safe, you’ll be required to comply with Accompany Health’s medical clearance requirements, including completing a TB screen and providing proof of immunity or vaccination for certain conditions. You will also be required to be vaccinated and up-to-date on your COVID-19 vaccinations, including boosters. This is a condition of employment, and we make exceptions as required by law.
