• We are looking for an energetic, independent and well-organized Registered Nurse to fill a Medical Case Manager position.
• 3 years of RN experience required, and active SOM RN license.
• This requires local travel to appointments. It is a direct hire position.

OVERVIEW

• As a Medical Case Manager you will make a meaningful difference in the lives of injured workers and their families.
• Your responsibilities include working closely with injured workers to facilitate their recovery.
• Work collaboratively with the patient, their family, medical providers, members of our team, and others.
• Compensation:: Starting salary is based on experience -$75K – $85K; Monthly and quarterly bonus potential.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES:

• Work from home, and on the road. Monday – Friday (8am – 5pm)
• Provides Medical Case Management to individuals through in person and telephonic communications with the patient, physician, other health care providers, employer and others.
• Evaluates patient’s treatment plan for appropriateness, medical necessity, and cost effectiveness.
• Provides assessment, planning, implementation and evaluation of patient’s progress.
• Negotiation the delivery of durable medical equipment and nursing services.

MINIMAL QUALIFICATIONS

• 3 years of clinical nursing experience in a clinical, acute/post-acute care, and community setting required
• 1 year of case management experience in a managed care setting strongly preferred
• Experience managing patients telephonically and via digital channels (mobile applications and messaging) preferred
• Registered Nursing Diploma or associate’s degree in nursing with an active Michigan License
• Reliable transportation in all four seasons.

CERTIFICATES, LICENSES, REGISTRATIONS:

• Current, active, and unrestricted Michigan Registered Nurse license required
• Certification in Chronic Care Professional (CCP) preferred
• Proficient computer skills – MS Office (Word, Excel, Outlook) and use of multiple monitors
• Case Manager Certificate preferred (CCM, CIRS, or equivalent); Must be willing to obtain within 36 months

Job Type: Full-time

Pay: $75,000.00 – $85,000.00 per year
