Remote Nurse Practitioner

• Conduct thorough health risk assessments, medication reviews, and cognitive screenings, while empowering patients with actionable education regarding their well-being.
• Manage preventive care interventions, handle referrals to specialists or community resources, oversee medication management, and thoroughly document all clinical findings.
• 6-8 patients per day, annual wellness exams, all ages.

Schedule:

Monday-Friday 7:30am-4pm (PST)

Requirements:

• Board Certified as a Nurse Practitioner
• Active Nurse Practitioner License in California
• Active California Furnishing License / Number
• Credentialed Medicaid provider for the state of California (Medi-Cal)
• 1.5+ years of clinical experience in family practice, primary care, or internal medicine
• Must have some telehealth (video visit) experience
