Join our dynamic healthcare team as a Remote Care Coordinator specializing in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM). Responsibilities

  • Conduct thorough patient assessments through electronic health records (EHR) and telecommunication tools, including vital signs and symptom review
  • Manage patient care plans by coordinating with healthcare professionals across hospital, outpatient, and post-acute settings
  • Monitor patient progress using RPM technology, identifying early warning signs of deterioration or complications
  • Educate patients on disease management, medication adherence, and lifestyle modifications to promote self-care
  • Facilitate discharge planning, follow-up appointments, and ongoing case management to ensure continuity of care
  • Document all interactions accurately within EHR systems while maintaining HIPAA compliance and safeguarding patient confidentiality
  • Collaborate effectively with physicians, nurses, social workers, and other healthcare team members to optimize patient outcomes

Experience

No prior Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) experience is required—we provide paid training.

Preferred Qualifications:

  • Medical Assistant (MA) certification or Licensed Vocational Nurse (LVN) license preferred.
  • Strong communication and customer service skills.
  • Comfortable speaking with patients over the phone.
  • Basic computer skills and ability to navigate multiple software systems.
  • Excellent organizational and time management skills.
  • Ability to work independently while collaborating with a remote team.
  • Compassionate, dependable, and committed to providing exceptional patient care.

Experience Level:

  • Entry level welcome.
  • New graduates are encouraged to apply.
  • Previous experience in a medical office, primary care, care coordination, CCM, or RPM is a plus but not required.
