Role Overview
Our client is looking for a Government Programs Nurse Case Management Senior Analyst) to support SNP members through thoughtful, high-quality care management. In this role, you’ll help improve health outcomes for members while also supporting those experiencing illness or injury.
You’ll work within a defined population and caseload, using the care management process to assess needs, coordinate services, and help members access the right care at the right time.
This role is best suited for someone who is comfortable working independently in a telephonic environment and who brings strong communication skills, clinical experience, and a passion for patient advocacy.
What You’ll Do
- Build collaborative relationships with members, families, physicians, and care teams to understand medical history, current health status, and care needs
- Promote member education and health advocacy to support better outcomes
- Assess health status and treatment plans to identify gaps or barriers to care
- Develop and document patient-centered care management plans with all relevant stakeholders (members, physicians, providers, employers, etc.)
- Identify expected outcomes, criteria for case closure, and ensure strong communication across all parties
- Implement, coordinate, monitor, and evaluate care plans on an ongoing basis
- Follow professional nursing standards, licensure requirements, and care management policies and procedures
- Participate in training and continuing education to maintain clinical expertise and certification
- Work with sensitivity and respect for culturally diverse members and communities
- All care management activities and performance metrics are monitored
Schedule
- 40-hour work week
- Monday–Friday, 8 hours per day (excluding company holidays)
What We’re Looking For
- Active, unrestricted Registered Nurse (RN) license in any U.S. state or territory
- Compact (multi-state) RN license required
- At least 2 years of full-time direct clinical care experience
