Are you passionate about patient care and creating a positive first experience for those navigating chronic health conditions? We’re looking for Care Coordinators (CTC) to join our growing team and play a key role in supporting the onboarding of patients into our Chronic Care Management (CCM) program.

About The Role

As a CTC, you’ll be responsible for educating patients on CCM services, obtaining consent, creating the first care plan, and ensuring a smooth relationship with your patients as you help then navigate through CCM. This is an excellent opportunity for someone who thrives in a collaborative, patient-centered environment and enjoys a mix of communication and administrative responsibilities.

What You’ll Do

  • Conduct initial outreach calls to introduce the CCM program and obtain patient consent
  • Develop and document the first care plan in alignment with CCM requirements
  • Complete the initial CCM report
  • Ensure all documentation is accurate, timely, and compliant with regulatory standards
  • Develop medical relationships with Patients
  • Address any questions or concerns patients may have during the onboarding process
  • Support various administrative tasks related to enrollment and care coordination

What We’re Looking For

  • Strong communication skills with a patient-first approach
  • Detail-oriented with excellent documentation and follow-up habits
  • Ability to work independently while contributing to a collaborative team
  • Experience in healthcare, care coordination, or customer service preferred

Schedule

  • Full-Time
  • Monday–Friday
  • 8 hour day- open to 4-10s Scheduling

  • Eligibility for Benefits: Begins on the 1st of the month following 60 days of employment

Compensation:

Compensation features supplemental pay: earn per patient served with a guaranteed minimum hourly wage of $16–$17, based on licensure and experience.
