Choice Health is a remote telehealth chronic care practice working with the elderly at home. We use CCM, RPM and monthly telehealth visits to keep our patients happy healthy and at home as long as possible. We also strive to help facilities reduce RTAs utilizing Transitional care management and a comprehensive continuity of care plan post discharge.
Role Description
This is a full-time/part time remote telehealth role for a Nurse Practitioner/Physician Assistant. The Nurse Provider will provide patient care services, including conducting assessments, diagnosing and managing acute and chronic health conditions, ordering and interpreting diagnostic tests, and prescribing medications. The role involves collaborating with healthcare professionals, ensuring patient education, and maintaining accurate medical records. The Nurse Practitioner will play a vital role in delivering compassionate, high-quality care through virtual patient interactions and telehealth services.
Qualifications
- Expertise in patient assessment, diagnostic evaluation, and clinical decision-making
- Knowledge of chronic disease management, medication prescribing, and treatment plans utilizing telehealth
- Strong written and verbal communication skills for patient education and clinical documentation
- Ability to collaborate effectively with healthcare teams in a remote work environment
- Proficiency with telehealth platforms and electronic health record (EHR) systems particularly ECW
- Active Nurse Practitioner (NP) or Physician Assistant license (PA) license and board certification in a relevant specialty
- Master’s or Doctorate degree in Nursing (MSN/DNP) from an accredited program
- Previous experience in primary care, urgent care, or telemedicine is a plus
