The Nurse Practitioner will be responsible for providing diagnostic patient care primarily through virtual consultations. This part-time role requires on-call coverage during weekends, focusing on assessing, diagnosing, and treating geriatric patients. The ideal candidate will have a strong background in telehealth and a commitment to delivering high-quality care. The position offers a pay range of $53.4 – $76.33 hourly, depending on experience and qualifications.
RESPONSIBILITIES
  • The candidate will be responsible for assessing acute and non-acute clinical problems through virtual consultations.
  • He/She will perform and document physical evaluations and patient histories, analyzing trends in patient conditions.
  • The role involves developing, documenting, and implementing a patient management plan based on interpretation of findings.
  • The Nurse Practitioner will aid in the development of a plan of care that may include health education and referrals.
  • He/She will evaluate the need for immediate nursing intervention and facilitate necessary patient care.
  • The candidate will plan patient care based on knowledge of the patient population and protocol.
  • He/She will serve as a patient advocate, collecting comprehensive health data.
  • The role requires analyzing data to determine appropriate health maintenance methods.
  • The Nurse Practitioner will confer with the patient’s PCP and other medical providers to report health data.
  • He/She will consult with patients and family members on health outcomes and work with them to maintain positive health habits.
  • The candidate will lead efforts to ensure optimal patient outcomes through Telemedicine.
  • He/She will collaborate with on-call PCP as needed to support expected clinical outcomes.
  • The role involves documenting assessments, interventions, and progress toward outcomes in an understandable format.
  • The Nurse Practitioner will work with key contributors to enhance telehealth practices through data utilization.
  • He/She will communicate using various formats to build professional relationships and deliver care.
  • The candidate will utilize appropriate resources to provide safe and effective services.
  • He/She may participate in clinical rounds and relevant meetings.
EDUCATION
  • A Bachelor’s degree in Nursing (BSN) and a graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty is required.
  • A Master’s degree in Nursing is required.
  • A valid, active Registered Nurse multistate compact license is required.
  • Board certification by AANP or ANCC is required.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required.
SKILLS
  • The role requires expert knowledge and understanding of medical practices to function independently as a certified practitioner.
  • Demonstrated record of consistently achieving clinical performance metrics is essential.
  • Technical capability to conduct telemedicine visits in accordance with regulations is necessary.
  • Excellent clinical judgement and problem-solving abilities are required.
  • The candidate must be able to prioritize and work under pressure, providing constructive feedback when needed.
  • Strong communication and collaboration skills with physicians, patients, and team members are essential.
  • Proficiency in Microsoft Office Suite products including Excel, Word, PowerPoint, and Outlook is required.
