The Nurse Practitioner will provide diagnostic patient care primarily through virtual consultations, serving as the dispositional authority for after-hours and weekend clinical calls. This role requires assessing, diagnosing, treating, and documenting patients’ health status while collaborating with other medical providers to ensure optimal patient outcomes. The position involves a rotating weekend schedule and requires adherence to state laws regarding onsite observations.
RESPONSIBILITIES
  • The candidate will be responsible for assessing acute and non-acute clinical problems through virtual consultations.
  • They will perform and document physical evaluations and patient histories, analyzing trends in patient conditions.
  • The role involves developing, documenting, and implementing a patient management plan based on interpretation of findings.
  • The candidate will aid in the development of a plan of care that may include health education and referrals.
  • They will plan patient care considering the patient’s cultural background and support systems.
  • The candidate will serve as a patient advocate, collecting comprehensive health data.
  • They will analyze data to determine appropriate health maintenance methods.
  • The role requires conferring with the patient’s PCP and other medical providers to report health data.
  • The candidate will ensure optimal patient outcomes through the use of Telemedicine.
  • They will collaborate with on-call PCPs to support expected clinical outcomes.

EDUCATION

  • A bachelor’s degree in Nursing (BSN) and a graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty is required.
  • A master’s degree in Nursing is required.
  • Board certification by AANP or ANCC is required.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required upon hire.

SKILLS

  • The role requires expert knowledge and understanding of medical practices to function independently as a certified practitioner.
  • Demonstrated record of consistently achieving clinical performance metrics is essential.
  • Technical capability to conduct telemedicine visits in accordance with state and federal regulations is required.
  • Excellent clinical judgement and problem-solving abilities are necessary.
  • The candidate must be proficient in Microsoft Office Suite products including Excel, Word, PowerPoint, and Outlook.
