Position Overview

A Remote Patient Monitoring Nurse is responsible for monitoring and responding to critical and at-risk patient readings, ensuring the accuracy of device usage, and making outbound calls to patients. This role involves screening for accurate readings and assisting with troubleshooting or submitting support tickets when necessary and is a vital role supporting patient care by addressing urgent health alerts and providing timely interventions.

Key Responsibilities:

  • Provide high quality patient education and coaching
  • Promote patient engagement via telephonic assessment, follow-up and personalized education
  • Triage “at risk” and “critical” readings and prioritize patient calls using sound clinical judgment
  • Build relationships with our partner physicians, ACOs, and Hospitals to establish and adhere to the continuum of care processes set in place
  • Monitor patient vitals and report concerns to the supervisor as necessary
  • Meet daily call quota to patients with critical and at-risk readings
  • Supervise and delegate appropriately to Medical assistants
  • Assist patients with telemonitoring services through telephonic assessment and education
  • Contact physician/providers as needed to report clinical concerns
  • Relay information back to the internal team
  • Complete focused clinical assessments and interventions for patients with critical and at-risk readings
  • Identify and troubleshoot device issues/user errors with patients
  • Other duties as assigned

Qualifications:

  • Active, unrestricted LVN, LPN or RN license, required
  • Graduate of an accredited LVN, LPN or RN program, required
  • 2 years of clinical experience, required
  • Minimum internet speed of 35 Mbps download and 10 Mbps upload, required
  • Ability to work from home in a HIPAA compliant environment, required
  • Bilingual proficiency in Spanish and English, preferred
  • Proficient using Electronic Medical Records (EMR) or Electronic Health Record (EHR) systems, required
  • Understanding of clinical workflows, quality assurance processes, and compliance standards
  • Excellent verbal and written communication skills
  • Strong organizational skills with attention to detail
  • Ability to manage multiple priorities simultaneously
