Job Title: Registered Nurse Care Coordinator

 

Location: In home visit locations vary, most work is performed remotely. Must reside in the Charlotte area in NC and be able to regularly visit areas in Charlotte and Union county for assessments.

 

Position Type: Mostly flex time with in home visits for new clients and then every 90 days. Should be available to respond to phone/text inquiries from families and caregivers in a reasonable time frame.

Job Summary:

Are you a Registered Nurse with a passion for ensuring high-quality care for senior clients in their homes?

We are seeking a dedicated and compassionate Registered Nurse Care Coordinator to be the primary point of contact for our clients and their families, ensuring they receive optimal care. In this role, you will coordinate care plans, manage communication with clients and staff, and oversee the delivery of care to ensure it meets our high standards.

 

Key Responsibilities:

  • Serve as the primary point of contact for clients and their families via multiple communication channels and addressing any questions, concerns, or requests.
  • Work closely with the scheduling department to modify and maintain the client’s care schedule, making adjustments as needed.
  • Coordinate comprehensive care for each client among all team members involved in their care.
  • Maintain accurate documentation of all client and caregiver interactions.
  • Conduct timely 90-day care plan reviews and nursing reassessments.
  • Supervise CNA staff and ensure they provide exceptional care.
  • Observe, assess, and document patient symptoms and progress, notifying the HIPAA representative if there are any changes in the patient’s condition.
  • Ensure the completion of all patient care paperwork in an efficient and timely manner.
  • Monitor infection control practices in all areas and ensure compliance with policies.
  • Participate in quality improvement initiatives and contribute to the development of CNA orientation and training programs.

 

Qualifications:

  • Active Registered Nurse (R.N.) license.
  • Minimum of 2 years of nursing experience preferred.
  • Strong communication skills, both verbal and written, with the ability to maintain clear and accurate documentation.
  • Ability to build strong relationships with clients, caregivers, and staff.
  • Excellent organizational skills and the ability to handle multiple responsibilities simultaneously.
  • Experience working with seniors strongly preferred.

Compensation is $100 paid bi-weekly per assigned case and $100 per assessment.
