Maiden Lane Medical is a leading, patient-centered, multi-specialty medical practice with seven locations across New York City. Our team is dedicated to delivering exceptional, evidence-based care through both in-person and virtual services. We are seeking an experienced Registered Nurse (RN) Care Coordinator to join our growing women’s health team.
Position Summary
The RN Care Coordinator serves as the primary clinical liaison between patients and providers in a fast-paced, high-volume women’s health practice. This role is responsible for managing telephone and patient portal triage, coordinating complex clinical workflows, tracking diagnostic testing and treatment plans, and ensuring timely follow-up for patients across multiple specialties.
This position is primarily focused on ambulatory care coordination, clinical triage, patient education, and longitudinal care management rather than direct bedside nursing care. The ideal candidate possesses excellent clinical judgment, strong organizational skills, and the ability to independently manage a large volume of patient communications while maintaining exceptional patient satisfaction.
Key Responsibilities
Clinical Triage and Patient Communication
- Manage high-volume patient communication queues through telephone encounters, patient portal messages, and electronic task management systems.
- Conduct structured clinical assessments and triage for gynecologic concerns, abnormal bleeding, pelvic pain, infections, medication questions, post-procedure symptoms, abnormal test results, and other patient-reported issues.
- Utilize evidence-based triage protocols to determine appropriate disposition, including home care recommendations, urgent appointments, provider review, or emergency evaluation.
- Identify urgent or emergent clinical concerns and initiate appropriate escalation protocols.
- Accurately document all patient interactions, assessments, recommendations, and follow-up plans within the Electronic Health Record (EHR).
Care Coordination & Clinical Workflow Management
- Coordinate diagnostic evaluations, treatment plans, and follow-up care for patients with complex gynecologic conditions.
- Facilitate scheduling and follow-up for colposcopies, LEEPs, endometrial biopsies, imaging studies, specialty consultations, and surgical procedures.
- Coordinate referrals and ensure completion of recommended diagnostic testing and treatment plans.
- Serve as a liaison between providers, specialists, imaging centers, laboratories, pharmacies, and external healthcare organizations.
Results Management
- Monitor, track, and coordinate timely follow-up of laboratory, imaging, pathology, and genetic testing results.
- Review results according to established clinical protocols and provider guidance.
- Communicate results and follow-up instructions to patients as directed by providers.
- Ensure abnormal findings are appropriately escalated and addressed in a timely manner.
- Patient Education and Counseling
- Provide patient education regarding diagnoses, medications, treatment plans, contraceptive options, preventive care, and pre- and post-procedure instructions.
- Reinforce provider recommendations and assist patients in understanding and navigating their care plans.
- Answer patient questions regarding treatment expectations, medication administration, recovery, and ongoing care. Administrative & Clinical Support
- Collaborate with administrative teams to facilitate prior authorizations, specialty referrals, peer-to-peer reviews, prescription refill requests, and urgent diagnostic testing.
- Maintain accurate, timely, and compliant documentation in accordance with organizational policies and HIPAA regulations.
- Utilize standardized workflows, templates, centralized queues, and task management systems to ensure efficient care coordination.
- Meet established quality metrics related to patient response times, documentation accuracy, care coordination outcomes, and patient satisfaction.
- Serve as a clinical resource for Medical Assistants and administrative staff regarding triage protocols and patient escalation pathways.
Qualifications
- Graduate of an accredited School of Nursing (ADN or BSN).
- BSN preferred.
- Active, unrestricted Registered Nurse (RN) license in the State of New York required.
Experience
- Minimum three (3) years of RN experience in gynecology, women’s health, ambulatory care, reproductive endocrinology, fertility, urogynecology, pelvic pain, or a related specialty.
- Prior experience in telephone triage, patient portal management, care coordination, or outpatient women’s health strongly preferred.
- Experience working in a high-volume, multi-provider practice preferred.
- Proficiency with Electronic Health Record systems such as eClinicalWorks, Epic, Athenahealth, or similar platforms.
- Strong computer skills and ability to manage multiple electronic communication channels simultaneously.
- Experience with virtual care workflows and remote patient communication preferred.

Schedule
- Full-time remote position.
- Monday through Friday schedule.
