Responsibilities

• Conduct thorough virtual patient assessments, including vital signs, medical history review, and physical examinations via telehealth platforms.
• Develop personalized treatment plans, prescribe medications within scope, and coordinate follow-up care to ensure optimal patient outcomes.
• Manage a wide range of medical conditions across specialties such as primary care, geriatrics, behavioral health, and chronic disease management.
• Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Athenahealth to document clinical encounters accurately and efficiently.
• Provide patient education on medication administration, lifestyle modifications, and preventive health measures to promote wellness.
• Collaborate with specialists, pharmacies, and other healthcare providers to coordinate comprehensive care delivery.
• Ensure compliance with HIPAA regulations and maintain strict confidentiality of patient information while adhering to telehealth best practices.

Skills

• Proven experience as a Nurse Practitioner with strong clinical skills in acute care, primary care, or specialty areas such as geriatrics or behavioral health.
• Extensive knowledge of medical terminology, physiology, and diagnostic evaluation techniques.
• Proficiency with telehealth platforms and EMR/EHR systems including Epic, Cerner, or Athenahealth.
• Ability to perform patient assessments remotely with excellent communication skills and empathy.
• Familiarity with CPT coding, ICD-10 coding standards for accurate documentation and billing processes.
• Experience working with diverse populations including children, seniors in assisted living or nursing homes, and individuals with disabilities or complex medical needs.
