Remote Medical Director / Nurse Practitioner – Remote Telehealth (CCM/RPM)

We are building a multi-state network of telemedicine clinics focused on Chronic Care Management (CCM) and Remote Patient Monitoring (RPM).

We are seeking licensed Nurse Practitioners and Physicians interested in a Medical Director–type role with a flexible, low time commitment.

What We Provide:

• We cover all startup and operational costs

• Full telehealth platform (EMR, AI tools, call center)

• Patient acquisition and scheduling

• Centralized team of NPs and licensed nurses handling day-to-day care

Role Overview:

• Clinical oversight of patients in CCM/RPM programs

• No requirement for high-volume patient visits

• Majority of care handled by support team

• Fully remote

Compensation:

• Guaranteed base compensation

• Additional upside tied to patient panel growth

Time Commitment:

• Approximately 2–4 hours per week

Requirements:

• Active NP or MD license

• Medicare credentialing preferred

• Interest in chronic care / telehealth
