What You’ll Do

• Speak with patients daily via phone to provide support and guidance
• Help patients understand their care plans and medications
• Coordinate follow-ups with providers and specialists
• Track patient progress and ensure no one falls through the cracks
• Build strong relationships with patients who rely on your support
• Document patient interactions in EHR systems

Qualifications

• Active LPN/LVN license (required)
