Remote Family Nurse Practitioner – Minnesota

PEAKSPAN is a growing health optimization and longevity company focused on helping patients improve their health, performance, body composition, and long-term wellness through personalized, evidence-based care.

We are seeking a motivated and patient-focused Family Nurse Practitioner (FNP) licensed in Minnesota to join PEAKSPAN as a part-time/contract telehealth provider. This is an opportunity to become involved with a growing practice from an early stage and help shape how we deliver personalized health optimization services.

The FNP will primarily conduct virtual patient evaluations, review medical histories and laboratory results, develop individualized treatment plans, provide patient education, and prescribe appropriate medications when medically indicated and within the provider’s scope of practice. The provider will maintain independent clinical judgment regarding all diagnoses, treatments, and prescribing decisions.

Our patient population will primarily consist of adults interested in weight management, metabolic health, longevity, physical performance, and overall health optimization.

Responsibilities include:

• Conduct telehealth new-patient evaluations and follow-up visits

• Review medical histories, medications, laboratory testing, and patient goals

• Order and interpret appropriate laboratory testing

• Develop individualized, evidence-based treatment plans

• Prescribe medications when clinically appropriate and legally permitted

• Provide guidance regarding nutrition, lifestyle, supplementation, exercise, and health optimization

• Monitor patient progress and adjust treatment plans when appropriate

• Maintain accurate and timely clinical documentation

• Communicate clearly with patients regarding treatment expectations, risks, benefits, and alternatives

• Follow applicable Minnesota laws, telehealth requirements, prescribing standards, and professional standards of care

Qualifications:

• Active and unrestricted Minnesota APRN/FNP license

• Current national board certification as a Family Nurse Practitioner

• Appropriate prescriptive authority

• Strong communication and patient-education skills

• Comfortable providing care through telehealth

• Strong interest in preventative health, metabolic health, weight management, longevity, and human performance

• Ability to exercise independent clinical judgment and prioritize patient safety

Experience in obesity medicine, metabolic health, primary care, sports/performance medicine, functional or integrative medicine, or telehealth is preferred but not required.

Position Structure:

This position is designed to begin on a flexible, part-time basis as PEAKSPAN builds its patient population. Compensation and scheduling can be structured around patient volume and clinical workload, making this an excellent opportunity for an FNP looking to supplement an existing position while becoming involved with a growing health optimization company.
