The Role
We’re hiring nurse practicioners as 1099 independent contractors to perform comprehensive telehealth evaluations for Family & Medical Leave Act (FMLA), state paid leave, and short term disability certifications. You’ll meet patients over secure video, perform a focused history and exam, determine medical necessity, and work with our operations team to complete Department of Labor form WH-380 (or employer equivalents).
You control your schedule, choose the visit mix that suits you, and are paid per encounter, regardless of whether the patient ultimately qualifies for leave. All clinical decisions remain 100% yours.
What You’ll Do
Deliver High-Integrity Tele-Visits
- Conduct 10-15 minute video exams with patients
- Evaluate cases relating to mental health, incapacity, chronic illness, etc.
- Use our dictation AI note scribe to approve patient FMLA findings & verdicts
Review FMLA Documentation
- Estimate incapacity duration or intermittent-leave cadence
- Never manually complete FMLA forms. Our operations team prepares all documentation, and you simply review and e-sign within 24 hrs.
- Maintain high responsiveness in reviewing forms and communicating with the team (via Slack or other channels)
Practice Evidence-Based Medicine
- Apply DSM-5 criteria for mental health cases
- Reference accepted guidelines for chronic and post-op conditions
- Educate patients on treatment plans & follow-up care
Uphold Clinical Quality
- Participate in quarterly peer-review meetings (remote)
- Follow our documentation checklist to ensure every form is “complete & sufficient”
- Report red flags or fraud concerns immediately
Collaborate with the Ops Team
- Provide input on clinical templates and telehealth workflow improvements
- Give feedback that shapes patient-facing FAQs and resources

Your Strengths
- Clinical Versatility – Board-certified in Family Medicine, Internal Medicine, Psychiatry, or PM&R; adept at evaluating both physical & mental health conditions.
- Documentation Mastery – You write concise yet thorough notes that easily withstand HR or DOL scrutiny.
- Multi-State Licensure – Active licenses in 10 or more states, specifically holding licenses in FL, TX, CA, NY, WA, NC, and VA.
- Tech Fluency – Comfortable in browser-based EMRs, e-fax, Docusign, and basic video troubleshooting.
- Time-Management – You can keep a 10-minutes visit on track without cutting corners and sign forms the same day.
What You’ll Bring
- Nure practitioner (APRN) with clean, unrestricted licenses in at least three states (more is better).
- Prior FMLA or disability-evaluation experience.
- Current board certification
- 3+ years of post-residency clinical practice (telehealth counts).
- Reliable broadband, webcam, quiet workspace, and comfort with 100 % remote care.
- Excellent written & spoken English.
Bonus Skills
- Spanish or other language proficiency.
- Familiarity with WH-380 variants and state paid-leave forms (CA CFRA, NY PFL, etc.).
Role Details & Requirements
- Commitment – 25–40 hrs/week (you pick fixed clinic blocks)
- Schedule – At least 2 hrs overlap with 9 am–7 pm ET weekdays
- Contract Type – 1099 independent contractor via physician-owned PC
- Location – Remote – U.S. only

Job Type: Contract
Pay: $90.00 – $120.00 per hour
