Role Description This is a full-time remote role for a Licensed Practical Nurse (LPN). The LPN will provide direct patient care via telehealth and other remote care platforms, including patient assessments, medication management support, and health education under the supervision of licensed providers. Daily responsibilities may include reviewing patient records, documenting clinical information, triaging patient concerns, coordinating care with physicians and other clinicians, and reinforcing care plans and follow-up instructions. The LPN will collaborate with the care team to promote safety, adherence to clinical protocols, and positive patient experiences, while maintaining strict confidentiality and compliance with applicable healthcare regulations.

Qualifications

 

  • Active and unrestricted Licensed Practical Nurse (LPN) license in the applicable state(s).
  • Experience providing patient-centered care in clinical, home health, or telehealth settings.
  • Strong skills in clinical documentation, electronic health records (EHR), and basic computer proficiency.
  • Effective communication skills, including the ability to explain medical information clearly and empathetically to patients and families.
  • Ability to work independently in a remote environment, manage time effectively, and prioritize multiple tasks.
  • Knowledge of HIPAA requirements and commitment to maintaining patient privacy and data security.
  • Comfort with technology and virtual communication tools; experience with telehealth platforms is a plus.
  • CPR certification and familiarity with standard LPN procedures and protocols.
  • High school diploma or equivalent required; completion of an accredited LPN program is mandatory.
  • Previous experience in primary care, chronic disease management, or similar medical practice settings is preferred.
