A fast-growing mobile and virtual health team in California is hiring an FNP, ANP, or AGPCNP to lead virtual annual wellness visits and health risk assessments for adult and geriatric Medicaid patients, while an in-home clinical team member handles the hands-on components at the patient’s residence. Monday through Friday, no nights, no weekends, no on-call, no standing panel, and no commute. Just focused preventive care through purpose-built technology at $145,000 flat, W-2, full benefits included.

Key Responsibilities

• Conduct virtual annual wellness visits and health risk assessments for adult and geriatric Medicaid patients
• Perform medication reviews, cognitive screenings, and preventive interventions
• Partner in real time with in-home clinical staff to evaluate vitals and complete hands-on visit components
• Coordinate care including specialist referrals, community resources, and follow-up documentation
• Chart accurately in a modern EHR while managing multiple digital tools simultaneously
• Build trust with a diverse, often elderly, patient population being seen in their homes

Qualifications

Required (no flexibility):

• Active, unrestricted California FNP-BC, ANP, or AGPCNP license (board certified)
• Active Medi-Cal provider enrollment in California (required at hire; actively enrolling considered)
• Minimum 2 years NP experience with at least 1 year of hands-on clinical work (hard requirement; no exceptions)
• Primary care background with direct experience conducting annual wellness visits for Medicare or Medicaid populations
• Reliable high-speed internet and private, HIPAA-compliant home workspace
• Comfortable working from a laptop all day using video visit tools and HIPAA-secure messaging

Preferred:

• Multi-state APRN licensure (compact or additional individual states)
• Prior telehealth, mobile health, or startup clinical environment experience
• Comfort with ambiguity and pace of an early-stage, high-growth clinical team

Schedule & Compensation

• $145,000/year | Full-time W-2
• Monday–Friday | Five 8-hour shifts | No nights, weekends, or on-call
