Full Job Description

Description:The Nurse Practitioner position is responsible for providing primary care services to participants of Kannact’s Chronic Disease Management Program including but not limited to: conducting comprehensive patient assessments, monitoring and evaluating ongoing patient conditions, contributing evidence-based care recommendations, promoting holistic care interventions, and working in collaboration with the patient health coaches to manage chronic disease, prevent disease exacerbations, and improve health outcomes. This position reports to the Medical Director and works closely with other Kannact Health Coaches in a team approach to participant care.

. Requirements: 

  • Assess patient status by obtaining a thorough health history through chart reviews of participants’ presenting illness, risk factors, family history, psychosocial situation, and cultural factors
  • Develop and implement treatment plans by considering what the participant’s doctor has prescribed in compliance with up-to-date practice guidelines and state laws
  • Educate participants regarding health promotion/disease prevention
  • Recommend and refer for appropriate community resources to meet patient/family needs
  • Communicate appropriate case management information to other members of patient care team
  • Prepare documentation for medical records including updating participant medical chart by posting results, diagnosis, medications, and treatment in computerized manner
  • Participate in staff education and other continuing education and quality assurance to demonstrate compliance with standards, regulations, policies, and procedures.
  • Act as collaborator to Health Coach team
  • Comply with participant’s confidentiality requirements
  • Troubleshoots technology issues effectively with little to no assistance
  • Attends staff meetings and trainings

 


Success Profile

  • Master’s Degree in Nursing or higher
  • Advanced Practice Registered Nurse (APRN) Licensure
  • Nurse Practitioner Certification appropriate for care of adult population
  • Preferred background/experience in infectious disease/internal medicine
  • Detail oriented and organized
  • Flexible/Adaptable to frequent changes
