Overview
Our client is seeking a Remote Advanced Practice Clinician (Nurse Practitioner) to provide telehealth urgent care services as part of a comprehensive, integrated care delivery program. This role focuses on delivering virtual care, coordinating with interdisciplinary teams, and supporting patients across multiple care settings.
This position offers the opportunity to work collaboratively while delivering high-quality, patient-centered care. The role requires flexibility, as clinicians support 24/7 coverage across multiple regions and patient populations.
Key Responsibilities
- Provide telephonic and video-based care to patients, caregivers, and facility staff
- Coordinate and manage care across providers, facilities, hospitals, and care teams
- Utilize EMR systems to document and deliver acute care services
- Conduct comprehensive patient assessments and document findings in compliance with regulatory requirements
- Manage acute, chronic, medical, and behavioral health conditions in collaboration with physicians or specialists
- Ensure accurate and complete documentation to support diagnoses and quality measures
- Apply evidence-based practice guidelines in care delivery
- Coordinate care transitions across various settings and adjust care plans as needed
- Monitor patient needs and evaluate care effectiveness and cost-efficiency
- Facilitate advanced care planning discussions with patients and families
- Communicate effectively with interdisciplinary teams, patients, and caregivers
- Participate in training, quality initiatives, and program improvement efforts
- Maintain a secure and private work environment to protect patient privacy
Required Qualifications
- Active and unrestricted Nurse Practitioner license in the state of residence and ability to obtain additional state licenses as needed (OH NP License Required)
- Compliance with state requirements regarding autonomous practice, if applicable
- Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program
- Active Nurse Practitioner board certification (AANP or ANCC) in one of the following: Family, Adult, Gerontology and Adult-Gerontology Acute Care
- Current DEA license/prescriptive authority or ability to obtain per state regulations
- Ability to work scheduled shifts, including nights, weekends, and holidays
- Proficiency with computers and electronic medical records documentation
- Ability to pass pre-employment requirements including drug screening and other regulatory checks
Preferred Qualifications
- 3+ years of clinical experience as an Advanced Practice Clinician
- Experience managing patients with complex behavioral, social, or functional needs
- Experience in nursing homes or acute care settings, particularly with senior populations
- Knowledge of geriatrics, chronic illness, and acute disease management
- Experience with advanced illness and end-of-life care discussions
- Strong interpersonal and customer service skills
- Adaptability and ability to work in a dynamic environment
Certifications/Licensure
- Active Nurse Practitioner license
- National Board Certification (AANP or ANCC)
- DEA license or ability to obtain
